Community Integrated Neurology and Stroke Service

We deliver an integrated, responsive, specialist rehabilitation service in the most appropriate environment for patients with acute neurological problems, long term neurological conditions and stroke survivors.

Our team will deliver the right care, in the right place, at the right time. We draw on the skills of professions including physiotherapists, occupational therapists, dieticians, neuro-rehabilitation consultants and psychologists within our team of around 50 staff.

We believe in setting joint goals with our patients. We set clear expectations from the start regarding what their treatment will entail.

Patients move towards ownership of a self-management rehabilitation plan and are also involved in the process that leads to them being discharged from our service.

Once known to the service, patients can refer themselves back to the CINSS team should their needs change after discharge. Patients are provided with clear information about how to do this when they are discharged.

The CINSS team work together with other services across LPT and external health and social care agencies (including the voluntary sector) in order to avoid duplication and deliver cohesive, patient-centred care.

The CINSS team provide care in the patient’s home, or from outpatient clinics at the following locations:

• Neville Centre (Leicester General Hospital site)

• Melton Hospital

• St. Luke’s Hospital, Market Harborough

• Hinckley and Bosworth Community Hospital

• Coalville Community Hospital

• Loughborough Hospital

• Rutland Memorial Hospital.

How can you refer?

CINSS referrals, in the form of a letter or a referral form (July 2019), are received via the single point of access (SPA).

All referrals are directed to our CINSS Hub based at the Braunstone Health and Social Care Centre, via SystmOne.

Clinical triagers then review the referral and ensure the referral generates the appropriate professional assessments.

This may be via phone assessment or a face-to-face visit. Triagers may also use information on SystmOne and seek additional information from referrers/patients to determine the appropriate location and urgency for the patient assessment.

Patients will be kept informed of who they will be seen by, when and where.
