Diana Children's Community Service - Short Break Care and Continuing Care Service

What Does Our Service Do?

The Diana Service offers families with children/young people with life limiting or life threatening conditions short breaks or continuing care.  These are planned visits providing opportunity for a family to have a break from the daily care routine of looking after their child / young person.  For the more complex child / young person these visits are aimed at reducing the pressure on the family of caring for their child with complex health care needs and an attempt to help minimise the frequency of hospital admission.
To receive either level of service the child / young person’s health care need’s will be formally assessed by a registered nurse.  They will complete with parent/s or carer/s and a key professional (as appropriate) a continuing care asseessment document.  This assessment will then be submitted and reviewed.
Health Care Worker’s will mainly provide the service supported by a named Diana Nurse. 

How Does Our Service Work?

A family receiving short break care / continuing care service can expect an allocated named nurse with a small team of health care workers to provide their child / young person’s care.  In turn they will together manage and deliver the scheduled care package.
A ‘getting-to-know’ process will be commenced so that the health care worker will know the child / young person well enough for them to be expected to undertake assessments in the health care tasks required to provide care for them, these are updated yearly.
If the continuing care assessment for short break care / continuing care is successful, the service will set up a package of care as the service capacity allows.  Sometimes, for continuing care service provision, the Diana service would need to employ further staff to be able to offer a family support.

There is an experienced pool of staff available to provide care in the attempt to meet demand for this service. 

  • This care will mainly be provided from the home or within an appropriate community setting (as arranged).
  • Social care residential settings

How Can You Access Our Service?

To be referred to our service the child / young person must be registered with a Leicester, Leicestershire or Rutland general practitioner. A referral to receive short break care / continuing care service can come from any professional (e.g. Health, Social, Education) involved with the child / young person.

There is a set criteria for accepting a referral for either service.  If a child / young person is not eligible the Diana Service we will endeavour to sign post the family to other services that may be able to offer support.
