Diana Children’s Community Service –End of life Care 24/7 On-Call


What Does Our Service Do?

The Diana Service provides an On-Call facility for children and young people at the end of life. The term ‘end of life’ refers to the final weeks of your child’s illness. This will be predicted by key professionals when your child shows significant signs of deterioration in their condition.

The On-Call is supported by registered childrens nurses who speak with you on a regular basis to determine the care needed. The nurses visit regularly but are not present in your home all of the time. Out of hours nurses can be contacted and will come and visit you at home if it is needed.

The Diana End of Life On-Call Service is activated for individual children at this stage of their illness.

This service is a 24 hours a day/ 7days a week provision provided by experienced qualified children’s nurses. They will endeavour to set up a package of care specific to your child’s individual needs. This care will be provided within your home or within an appropriate community setting.

The Diana Service will provide planned visits, visits when your child’s symptoms change and offer support when your child dies.

How Does Our Service Work?

The Diana Service End of Life 24/7 On-Call is to provide a high standard of nursing care to children, Young people and their families in their home or appropriate community setting. The service endeavours to offer each family the choice of where they wish their child to die, in order for those closest to the child to have time together.

The Diana Service will endeavour to accept all referrals for End of Life Care. Due to the complexity of managing End of Life Care the service is able to accept a maximum of two children/ young people for 24/7 On-Call provision at any one time.

Due to the complexity of the care provided and to support our local loan working policy two nurses will visit you as required.

This care will be provided within your home or within an appropriate community setting.

How Can You Access Our Service?

To access our service you can:

  • Speak to your GP
  • Speak to any other healthcare professional involved in your care

The Diana Service will accept referrals for 24/7 End of Life On-Call within office hours Monday-Friday (before midday). The team requires 48 hours notice prior to commencing the provision however will endeavour to activate this sooner if resources are available. Following the referral the Diana Nurses will assess the child/young person’s condition and discuss the criteria for acceptance with the referring professional.
